Personalized Front door plaque?

I'm trying to find a website that can make a personalized plaque for my front door. I want it to say something like "The Smith's", preferably made out of the broken tiles. I've been searching, but I can't find anything like that. Perhaps I'm not phrasing it correctly. If anyone knows of a good site for this, I would really appreciate it!
